Lilly starts most men at 10 mg taken before anticipated sexual activity. The dose may rise to 20 mg or fall to 5 mg based on effect and how the tablet sits. Once per calendar day is the ceiling. A second tablet the same night is outside the label.
Food does not move that clock. Cialis may be taken without regard to meals, which is why this lock is easier to place on a Friday than sildenafil, whose peak slips after a heavy plate. The trade is duration. Whatever you feel, good or sour, tends to stay longer.
Sexual stimulation still has to start the nitric-oxide signal. Tadalafil keeps cyclic GMP from being broken down by PDE5. It does not manufacture desire and it does not raise an erection in an empty room. Men who swallow 20 mg and wait in silence often call that a failed pill. It was never built to fire itself.
Reviews that treat 10 mg and 20 mg as interchangeable miss the titration. Ten is the usual first issue. Twenty is the labelled ceiling for as-needed use. Five is the step down when headache or backache outweighs the gain. The daily-versus-on-demand briefing covers the other rhythm. This page stays on the PRN pair.
Matching the script to the reviewed milligram is the only honest fill. I will not issue 20 mg because a cart sold a thirty-count cheaper. Ten milligrams reviewed stays ten milligrams at the window.
Daily 2.5/5 mg and the BPH line sit on another card
Daily tadalafil is a different lock. The labelled once-daily start for erectile dysfunction is 2.5 mg at about the same time each day, without tying the swallow to sex. The dose may rise to 5 mg. That pack is not what this page prices.
BPH at 5 mg once daily is the urinary indication. The same 5 mg tablet is the labelled dose when ED and BPH are treated together. Alpha-blockers used for prostate symptoms should not be stacked with tadalafil for BPH. The combination has not been adequately studied for that use, and both lower blood pressure.
Combining tadalafil with an alpha-blocker for ED is a caution, not an automatic ban. The man should already be stable on the alpha-blocker. Tadalafil then starts at the lowest recommended dose. Starting both on the same day is how people faint in a bathroom.
Adcirca 40 mg is two 20 mg tablets taken together once daily for pulmonary arterial hypertension. It is the same molecule and a different product. Do not add Cialis on top of Adcirca. Do not treat a PAH bottle as leftover ED stock. If PAH is the question, this sheet is the wrong issue.
Lilly's once-daily ED start is 2.5 mg. BPH, alone or with ED, is 5 mg at about the same clock time each day. Those packs look similar in a photo. They are not this page's cash band. Ordering the wrong NDC is how a weekend plan becomes an accidental daily course.
Why the 36-hour window changes the evening
A 36-hour response window is the measurement that made Cialis famous. The US label states that as-needed tadalafil improves erectile function compared with placebo up to 36 hours after the dose. That is a plateau, not a guarantee that every hour 12 through 36 will feel like hour 2.
Couples who hate watching a clock often pick this lock for that reason. One tablet covers a night and the next morning without a second swallow. The same stretch is why a nitrate emergency is harder to manage. The drug is still present long after the evening ends.
Median time to peak is 2 hours, with a range from 30 minutes to 6 hours. Some men notice an effect nearer 30 minutes. Others need the full two hours. Planning around the median is more honest than promising a 20-minute onset.
Side effects, if they appear, follow that same long curve. Back pain and myalgia in the pharmacology studies often began 12 to 24 hours after the dose and settled within 48 hours. That is late enough that people blame a mattress. It is the half-life talking.
Primary US and non-US as-needed trials used IIEF erectile-function scores and SEP diaries, not a stopwatch. Men chose their own interval between the tablet and the attempt. Food and alcohol were not restricted. The 36-hour claim is that comparison versus placebo, not a promise that hour 30 equals hour 2.

Reviews that advertise a no-prescription cart are not a price win. In the United States this remains a prescription medicine. A consult that never asks about nitrates is not a consult. Counterfeit 20 mg tablets are a known market.
ZIP code moves the print. Insurance may ignore the cash coupon entirely. Ask the window to price the prescribed strength and count, not a leftover bottle from another indication. If the script is 10 mg, do not pay against a 20 mg board and split tablets unless the prescriber wrote that plan.

How the tablet sits: 17.5 hours and a 63 L volume
Mean terminal half-life in healthy subjects is 17.5 hours. Mean oral clearance is 2.5 L/hr. Those two numbers are why a Friday 20 mg is still in the system on Sunday morning. They are also why this lock is a poor match for anyone who might need a nitrate over the weekend.
Protein binding sits near 94 percent at therapeutic concentrations. Apparent volume of distribution after an oral dose is about 63 L. Absolute bioavailability has not been determined. Less than 0.0005 percent of a dose showed up in semen in healthy men.
Metabolism runs mainly through CYP3A4 to a catechol, then to a methylcatechol glucuronide. The circulating metabolites are not expected to carry the clinical effect at observed levels. Excretion is as metabolites, mostly in feces, with a smaller urinary fraction.
A 20 mg dose in a clinical pharmacology study dropped supine blood pressure by a mean of 1.6/0.8 mm Hg versus placebo in healthy subjects. Most men will not feel that. Men with poor autonomic control of pressure can. Fitness for sex still matters more than the milligram stamp. Unstable angina, a recent infarct or stroke, and uncontrolled pressure keep this lock unissued.
Hearing loss, when it is sudden, is a stop. So is a dark field in one eye. Neither is a reason to take a second 20 mg. Both are reasons to say tadalafil was taken and when. Dr. Lena Hofer will not second-guess an emergency desk from [email protected].

Nitrates in any form are contraindicated. Regular, intermittent, spray, patch, or tablet. Cialis was shown to potentiate the hypotensive effect of nitrates. That is a never, not a 'usually avoid'. If chest pain hits after a dose, say tadalafil was taken and when. Do not self-rescue with nitroglycerin.
Riociguat sits on the same forbidden list. Guanylate cyclase stimulators plus a PDE5 inhibitor push the same pathway from two sides. Hypersensitivity to the tablet is the other hard stop.
Strong CYP3A4 inhibitors raise exposure. Ketoconazole, ritonavir, and itraconazole are the named examples. For as-needed use the cap is 10 mg, not more than once every 72 hours. For once-daily use the cap is 2.5 mg. Rifampin and other inducers can flatten the effect by clearing the drug too fast.
Creatinine clearance between 30 and 50 mL/min starts as-needed dosing at 5 mg, with a maximum of 10 mg no more often than every 48 hours. Below 30 mL/min, or on hemodialysis, the as-needed ceiling is 5 mg every 72 hours and daily use is not recommended. Severe hepatic impairment (Child-Pugh C) is not a tadalafil candidate on this label.
Child-Pugh A or B on-demand use is usually capped at 10 mg. Daily tadalafil in that liver group is a narrower path and is avoided in Class C. Alcohol at five units or more can add a standing-pressure drop on top of the tablet. That is a counseling line, not a second contraindication next to nitrates.
Headache, gut burn, and the late backache

Headache and dyspepsia show up most often on the as-needed 10 mg and 20 mg tables. At 20 mg, headache reached 15 percent and dyspepsia 10 percent in the pooled as-needed set. Nasal congestion, flushing, and pain in a limb sit lower, still above 2 percent and above placebo.
Back pain and myalgia track this molecule more than they track sildenafil or vardenafil. On 20 mg, back pain was 6 percent and myalgia 3 percent. The discomfort is often bilateral in the low back, gluteal belt, or thighs, worse when lying still, and late. About 0.5 percent of on-demand subjects stopped because of it.
Priapism is uncommon and still a reason to go to an emergency desk. An erection past 4 hours needs care. Painful erection past 6 hours is priapism. Sickle cell disease, myeloma, leukemia, and a bent or fibrotic penis raise the risk. Sudden vision loss in one or both eyes can be NAION. Stop the drug and get seen. The same rule applies to sudden hearing loss.
Alcohol at 5 units or more can add dizziness and a standing-pressure drop. Ordinary blood-pressure tablets usually stay compatible, with a small extra dip. The list still has to be read. Two PDE5 inhibitors in the same window is not a clever stack. It is a duplicate brake on the same enzyme.
